What nerves innervate the parietal pleura, and how is this important in assessing pain during breathing? |

The parietal pleura is innervated by intercostal nerves (costal and peripheral diaphragmatic pleura) and phrenic nerves (central diaphragmatic and mediastinal pleura). This is clinically important because pain from the parietal pleura is somatic and thus well-localized. It can be referred to the shoulder when the diaphragmatic pleura is irritated (C3-C5 dermatomes via the phrenic nerve) or to the thoracic wall for costal pleural irritation (via intercostal nerves). In contrast, the visceral pleura is insensitive to pain as it lacks somatic innervation. |
