CASE REPORT:

A 39 y/o carpenter, who is a recovering alcoholic, is being supported by outpatient rehabilitation by intragluteal injection of naltrexone, an opioid antagonist, every 4 weeks. Immediately after the last injection into his right buttock, he complained of numbness, tingling, and burning in his right leg down to his toes. The next day, he developed a foot-drop and was hospitalized.

EXAMINATION:

  • • Inspection of the right gluteal region shows an injection mark approximately over the course of the right sciatic nerve slightly above the gluteal fold.
  • • Sensory loss of the outer side of the right calf and the dorsum of the foot.
  • • Motor loss included inability to dorsiflex the ankle or evert the foot, with noticeable foot-drop.
  • • There is also difficulty in extending the toes.
  • • When walking the patient drags the front of the outer margin of his right foot.


  • What is the path of the sciatic nerve as it leaves the gluteal region.
